Kaua‘i Medical Clinic combines the intimacy of a neighborhood clinic with the resources of Hawai‘i’s largest multi-specialty medical group. The clinic currently has 88 physicians and more than 20 medical specialties. It provides the majority of primary and secondary care to Kaua‘i’s residents, serving as the most comprehensive medical practice on the island. Combined with the resources and financial strength of Hawai‘i Pacific Health, a multi-hospital and medical group system, we offer the benefits of working within a group practice. The main clinic is located on our affiliated Wilcox Medical Center campus, a modern 71-bed community hospital, and there are also three primary care satellite clinics in other locations around the island.
Hawai‘i Pacific Health is committed to community outreach and education. The Population Outreach team provides administrative support and clinical data monitoring to support Hawaii Health Partners, a physician-led accountable care organization with the goal of providing high quality care, increased efficiency and optimal patient health. We believe that greater clinical integration, collaboration and shared accountability among a network of independent physicians, employed Hawaii Pacific Health physicians, and Hawaii Pacific Health hospitals and clinics will create better health outcomes for the people of Hawaii.
As a Care Management Coordinator in our Population Outreach department, you will provide quality metrics support for primary care physicians under Hawai‘i Health Partners. In this role, you will review patients’ available data, including clinical/claims history, outpatient treatments, medications, medical benefits from electronic medical records (EMR), chart reviews, or other information to assist in monitoring and facilitation adherence to approved standards of practice and protocols. We are looking for someone detail-oriented and attentive, with strong communications and patient care skills and a commitment to creating a healthier Hawaii.
