Job Description
With 118 beds and more than 400 physicians on its medical staff, Pali Momi Medical Center offers a full range of services for the communities of Central and West O'ahu. The hospital has delivered many medical firsts for the community, including Central and West O'ahu’s only interventional cardiac catheterization unit to detect and treat heart disease, a fully integrated minimally invasive surgical suite, a comprehensive women’s center, CT scan and MRI services, and the state’s first retina center. Pali Momi is the only facility in Hawai'i that uses a team triage approach in its emergency room, where patients are promptly evaluated by an emergency physician.

Our family physicians are devoted to providing personal, comprehensive and continuing care for people of all ages in the context of the family and the community. These warm professionals emphasize disease prevention and health promotion with a strong foundation in primary care ethics, and are often best qualified to serve as the patient’s advocate in all health-related matters, including the appropriate use of consultants, health services and community resources.
As the Community Outreach Worker provides a wide variety of services to engage, support, advocate, and educate community members in overall health and wellness. You will serve as a liaison between health and social services in the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as agencies to provide short-term care coordination and connection to resources and support to programs for clients to improve their health and general well-being through educations and provision of coordination or care and services. This unique role works in both clinical and community-based settings, including clients homes. We are looking for someone detail-oriented and attentive, with strong communications and patient care skills and a commitment to creating a healthier Hawai‘i.

Minimum Qualifications: High School or equivalent. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required. One (1) year of experience in customer service or related field.
Preferred Qualifications: Experience in population health management, community outreach, community assessment or advocacy.

Hawai‘i Pacific Health offers a comprehensive and competitive total rewards package that includes pay and benefits. The pay rate, if displayed, is the minimum hourly rate for the position. Rate of pay for selected candidates will be determined by various factors including knowledge, skills, abilities, relevant experience and training, as well as internal peer equity.
Starting Hourly Rate: 18.66
