Job Description

Kaua‘i Medical Clinic combines the intimacy of a neighborhood clinic with the resources of Hawai‘i’s largest multi-specialty medical group. The clinic currently has 88 physicians and more than 20 medical specialties. It provides the majority of primary and secondary care to Kaua‘i’s residents, serving as the most comprehensive medical practice on the island. Combined with the resources and financial strength of Hawai‘i Pacific Health, a multi-hospital and medical group system, we offer the benefits of working within a group practice. The main clinic is located on our affiliated Wilcox Memorial Hospital campus, a modern 71-bed community hospital, and there are also three primary care satellite clinics in other locations around the island.

Comprehensive Primary Care Plus (CPC+) is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. The use of team-based care features uniquely skilled individuals working together, in new ways, to meet the health care needs of patients in an efficient, comprehensive, and sustainable manner. Successful population management relies on a high-functioning team that shares a firm vision of a healthier patient who is empowered to better manage his or her own health, with the help of the Care Team. These physician-led teams are aware of the patient’s medical and social history, and are equipped to attend to patient needs in a variety of ways. Team-based health care is one of the key ways to accomplish shared goals within and across our medical centers to achieve coordinated, high-quality care and reach our mission to create a healthier Hawai'i.

If you are compassionate, attentive and enjoy working with people, you could be the ideal candidate for our Nurse Practitioner position at our Outpatient Clinics, where you will help to provide our signature high quality patient care to patients and their families. You will be delivering direct treatment, evaluation and technical procedures to a specific group of patients in an assigned unit, including health education, referrals to other disciplines and consultation with colleagues and other allied health personnel. We are looking for someone professional and nurturing, with excellent communication skills a commitment to delivering the highest quality health care to Hawai‘i’s people.

Minimum Qualifications: Current Hawai‘i Registered Nurse license. Current Hawai‘i Advanced Practice Registered Nurse (APRN) license. Current Advanced Practice Registered Nurse (APRN) certification in area of specialty. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Two (2) years recent Ambulatory Care Registered Nurse experience.

Preferred Qualifications: One (1) year advanced practical experience.
